It has to be used within 1 day and let you reset any skills. It is once per character, so if you want to get it again, you will need to level up an alt again (class shouldn’t matter, can be the same class if you will). At least that’s how I did it, when I wanted a free reset.

You mean you learned the skill again and it doesn’t work at all or your hotkey doesnt work? Yes, that´s right. For example: I learn Darkness Burst from I to V which by default is R2, I reset my combat skills. Relearn the skill again, doesn´t work at all. The character doesn´t learn it and pressing R2 does nothing.

I think when you hit level 62, you can unlock all the skills, but you’ll need the points to do so aswell. You’ll need at least 1000+ skill points yo get all the awakening skills. (Probably more if you want all of the normal skills too.) I’d suggest finding a PvP/PvE/PvEvP skill guide for your class and sticking to it.
