I read a post today from another player related to disconnects. Somebody replied and suggested he try switching to DX9 in the options menu. I decided to give it a try. Nothing to lose since it was unplayable for me. I changed from DX11 to DX9 in the options menu, restarted and I played for about 90 minutes. I saw smooth gameplay and no disconnects.

There’s some kind of problem that occurs sometimes when loading the game. It will seem like it is taking forever and not-responding, so you have to click outside of the game (onto your second monitor) and then click back into the game and then all is well.
